Report: Navigator involved in nuke-powered US sub crash was listening to iPod

6515314 Report: Navigator involved in nuke powered US sub crash was listening to iPodWASHINGTON — A crash between a nuclear-powered US submarine and a warship in the strategic Strait of Hormuz was an "avoidable" accident caused by complacent sailors and weak leadership, a Navy report said.

The crash between the USS Hartford and USS New Orleans, an amphibious vessel, on March 20 was a "catastrophic failure" of management and navigation practices on the submarine, according to the report obtained by the Navy Times through a Freedom of Information Act request.

The investigation, reported by the Navy Times on Wednesday, revealed that sailors on board the Hartford took a "lax" approach to navigating their vessel, with those assigned to keep watch often sleeping on the job.

The heavily redacted report described one navigator listening to his iPod and simultaneously taking an exam while on duty.

"This incident comes down to weak and complacent leadership, which led to inadequate planning and preparation of the crew," the report said.

Admiral John Harvey called the incident "an avoidable mishap" as he endorsed the investigation's findings.

"Correction of any one of nearly 30 tactical and watchstander errors, or adherence to standard procedure, could have prevented this collision," he added.

"In this case, the command team failed to do so, and a high price has been paid for that shortcoming."

The crash injured 15 sailors and caused the fuel tank of the New Orleans to rupture, spilling 25,000 gallons (95,000 liters) of diesel fuel.

It caused 2.3 million dollars in damage to the New Orleans, and the Hartford is still undergoing extensive repairs for costs estimated at 100 million dollars.

The Navy investigation said the New Orleans crew "bears no fault" for the crash.

The Strait of Hormuz, less than 60 miles (100 kilometers) at its widest point, separates Oman from Iran and is the gateway into the oil-rich Gulf.

An estimated 40 percent of the world's crude oil passes through the strait on the way to market.

    "“Over several months” prior to the incident, hundreds of watchstanders were tested in their ability to understand how to analyze the movement of surface contacts. The exams yielded results of 10 percent to 15 percent passing grades among enlisted watchstanders and 60 percent of officers."

    10 to 15 percent! This means it's just a lucky guess if they are able to recognize another ship being on a colision course. Yeah, this is really inspiring confidence, isn't it???

  • Some more points, after reading the report: It's not true that the "Navy investigation said the New Orleans crew "bears no fault" for the crash". It says, most of the crew are excellent and shouldn't be punished. At the same time, it recommends punishments or actions against 19 (!) officers, supervisors and operators. All in all, the report shows that discipline was very lax on the boat, and crew members on watch couldn't be relied on to do their assigned duty. They could be on a totally different position, doing totally unrelated duties, chatting or listening to music, or even be asleep. In short, this was a pigsty of a Navy ship!

    One of the most devastating findings, imho, is that the crew not only failed to properly id and track targets, but also overly relied on AIS, an identification signal that's only used on civilian ships. And they failed to notice 24000 ton USS New Orleans coming dangerously close from straight ahead, even one minute before the colision, at a distance that must have been less than half a mile.

    Yeah, very impressive for the crew of a fast attack submarine which is designed to hunt warships, who typically don't give away their position with an AIS signal. And this, at the strait of Hormuz, near the coast of enemy nation Iran. If this would be typical for submarine crews, you could spare the about 3 billion dollars it costs to build such a boat, because it would then be totally useless.

    This Captain and to be honest the rest of his command team and command NCO (19 individuals that usually means the whole command staff for a sub of 135 people) careers are not only over but destroyed. I wouldn't be surprised if some court martials aren't in the pipeline right now. You see the interviewed the whole crew and everyone started to yell about this they saw and that they saw in order to not be implicated in this fiasco. And since it was so much, they considered the command being lax in their duties and in leadership. I was in the Navy for a time, and I can tell you a few things. When something big like this happens, the captain was going to get blamed regardless. You see the Navy needs a person to show the rest of the Navy and the civilian leaders that THIS person was responsible.

    Also OldAtlantic, it wasn't the lowering of standards. I went to the Navy boot camp in Orlando before it's closing and they had a nuke school located there. Many of the men and boys that was in my company going through bootcamp had billets to go to nuke school, many of them didn't make it. You have to pass a background check (you see the navy and the govt. doesn't want a person to walk away or talk about things that are secret too.), and if you had a speeding ticket you where booted pure and simple. People where finding out starting week 4 that they where denied this school and where put into open status for school (which means you get the worst job). Also the schooling required while going through nuke school was like taking a 4 year MIT degree in nuclear science or electronics and putting it in 2 1/2 years. There wasn't time to be "Hip-Hopping" or (they wouldn't allow you to have girlfriends also, because of the work load) gang this or that. They Navy meets it's "diversity quota" by having minorities in the navy and in alot of positions (to be honest many of the officers and the enlisted would like nothing better to NOT be on a ship and have commands and or billets on shore, period) on many ships and shore commands. They are picky when it requires people for there most critical weapons and such. What happened here was hubris and cockiness and outright lethargy. They where having it easy because the capt. was easy so they did stuff that you wouldn't be able to get away with on many navy ships.

    Also OldAtlantic, on many of your posts over the weeks and months you have a race slant you try to put out there. It seems it's always the minorities that are making this planet, this country, this whatever worse. But us whites aren't doing anything wrong are we? We just happen to be here and these hoards of dark people attack us, eat our food, date our women etc. etc. and they are to blame. I guess the indians that where in the US before the settlers came attacked europe and in so doing the Quakers decided to retaliate by taking after they found out that these "savages" wheren't a threat to them, and on and on and on.     You try to make racism logical by throwing up strawman theories and ideas and taking anything that gets news worthy with a dark face or a foreign name and make it into a reason to not have this or not have that. Racism isn't logical OldAtlantic, not by a longshot. There human just like you and me, we can have kids with them, they are intelligent, they can be POOR just like us, etc. etc.. Who built the pyramids OldAtlantic, I'm pretty sure it wasn't blond hair blue eyed people, Where did Algebra originate OldAtlantic, it wasn't a Blonde Hair Blue eyed people that made it it was from the middleeast, and on and on. You try to make us as the chosen ones because of our skin color (They used to call us Irish back in the day Patty and Spud N****rs and alot worse) being white. What your feeling and is logical is Tribalism, where a people that share a characteristic (doesn't have to be race, it can be religion, sex, size, etc.) and group to that group and without knowing it look out for a person they perceive as being one of their own.

    The problem with that is if you haven't notice, our "tribe" is getting smaller and smaller every year. And if you keep pushing this my characteristic is more important than those people's, you may be in for a horrible shock. A shock that one day you look around and see a world that you don't recognize because you spent your years pushing for a worldview that was slowly changing to include different people and races. But look on the bright side if your unhappy OldAtlantic, by the year 2100 us whites in the US my be the ones asking for affirmative action, doesn't life have it's ironies.

    Oh, there's a movie you should see that was sleeper hit back in 1995 called White Man's Burden. It starred John Travolta and Harry Belafonte, it was well done and made you think. After the first 10 minutes you will see why in the US it didn't have solid showing even with John Travolta coming off of the Pulp Fiction win.
